To request a New Patient Appointment or a Free Consultation, you can use our secure self-registration form below.

 

Once you have submitted the form, you will receive a welcome email to our patient portal in ClinicMind with a link and log-in code.  In the patient portal you will be able to schedule an appointment and complete the Health Intake Form before your New Patient Appointment.
